Obtain history

1. When in course of tx did sx begin?
2. What product was tx'd and how much?
3. Symptoms? (short of breath, itching, nausea, etc)
4. Signs? (fever, BP, O2 sats, rigors, hives, wheezing, crackles, red urine, CXR, etc)
5. Premedication?
6. Was blood prewarmed?
7. Anything besides NS given with products or in line?
8. Transfusion, IVDA, pregnancy history
9. Treatment for symptoms? Effective?

Hemolysis workup

1. Stop transfusion and send blood to blood bank
2. Clerical check
3. Draw blood in red and purple tops (for DAT and visual hemoglobin tests)
4. Maintain venous access
